How to get an Ayushman Bharat card: eligibility, documents and steps

Key takeaways

  • -PM-JAY covers hospitalisation costs up to a fixed annual limit per family at empanelled hospitals.
  • -Eligibility is based on government databases, not on application — you check whether you are already listed.
  • -The card itself is free. Nobody should charge you to create it.
  • -Cover applies only at empanelled hospitals, and only for hospitalisation, not routine OPD visits.

Ayushman Bharat Pradhan Mantri Jan Arogya Yojana, usually shortened to PM-JAY, is a government funded health scheme that pays for hospitalisation at empanelled hospitals. The most common confusion around it is simple: people think they need to apply for it. In most cases you do not apply. You check whether your household is already on the beneficiary list, and then you get the card made.

Who is eligible

Eligibility is decided from government databases rather than from a fresh application. Households identified through deprivation and occupational criteria in the socio-economic survey form the base list. Several states have widened this using their own welfare databases and ration card data, so the answer genuinely differs from state to state.

Because of this, the only reliable way to know is to run a check against your own mobile number or ration card, rather than relying on what a neighbour or an agent tells you.

How to check whether you are eligible

  1. Open the official beneficiary portal or the Ayushman app on your phone.
  2. Log in with your mobile number and the OTP sent to it.
  3. Select your state and choose a search method — family ID, ration card number, Aadhaar number or the name based search.
  4. If your household appears in the results, note the names listed against it. Each member needs their own card.

Documents you need

  • Aadhaar card of the person the card is being made for
  • A mobile number that can receive the OTP
  • Ration card or family ID, depending on what your state uses
  • A recent photograph in some cases, captured on the spot during e-KYC

Completing e-KYC and getting the card

Once your name is found, the next step is e-KYC. This can be done through the app using Aadhaar OTP or face authentication, at a Common Service Centre, or at the Ayushman help desk that empanelled hospitals are required to run. After e-KYC is approved, the card can be downloaded as a PDF and printed. Approval is sometimes instant and sometimes takes a few days if it goes for manual verification.

What the scheme covers

The cover is meant for secondary and tertiary hospitalisation. That includes the cost of the bed, surgery, medicines during admission, diagnostics related to the admission, and a defined period of pre and post hospitalisation expense. Treatment packages are pre-priced, which is why the hospital does not negotiate rates with you at admission.

What it does not cover

Routine outpatient visits, medicines bought outside an admission, cosmetic procedures and treatment at hospitals that are not empanelled fall outside the scheme. This last point causes the most trouble in practice. Families arrive at a hospital assuming the card will work, and find out at the billing counter that the hospital is not on the list.

Practical advice before an admission

  • Check the empanelled hospital list for your district before you need it, not during an emergency.
  • Carry the card and the Aadhaar of the patient to admission.
  • Ask for the Ayushman help desk by name — empanelled hospitals are expected to have one.
  • If cashless is refused at an empanelled hospital, ask for the reason in writing and escalate to the state health authority helpline.

Keeping a printed copy of the card with your other documents saves time in exactly the situation where you will not have time.

Frequently asked questions

Is there any fee for making an Ayushman card?

No. Creating the card is free at Common Service Centres, empanelled hospitals and through the official app. If someone asks for money to make your card, that is not an official charge.

Can I use the card at any hospital?

No. Cashless treatment works only at hospitals empanelled under the scheme. Check the empanelled list for your district before admission where possible.

What if my name is not in the beneficiary list?

Eligibility is drawn from government survey and welfare databases. If your name does not appear, contact your nearest Common Service Centre or the state health authority to check whether a correction or a different state scheme applies to you.

Does the card cover OPD consultations and medicines at home?

The scheme is designed around hospitalisation. Routine outpatient consultations and pharmacy purchases outside an admission are generally not covered.
